Hospital Compliance with SB 505 – Alternate IRS Form 990
Per the requirements of Senate Bill 505 (SB 505), for any hospital not required to file an IRS Form 990, the Georgia Department of Community Health (DCH) is required to establish and provide a form that collects the same information as is contained in the IRS Form 990, including Schedule H, for applicable hospitals. In compliance with the statute, DCH has developed the SB 505 990 Alternate Form for applicable hospitals to complete and post on their websites. Hospitals may utilize the Schedule H – Community Benefit Worksheets for calculations only and save the documents in their internal files. The worksheets should not be posted on websites with the 990 Alternate Form.
The Alternate Form and Worksheets will be posted on DCH’s Hospital Transparency website at: https://dch.georgia.gov/hospital-transparency-information . Questions may be submitted to the hospital transparency email box at dch.transparency@dch.ga.gov .

An overdose is a poisoning. Call the Georgia Poison Center and immediately speak to an Addiction Medicine specialist for assistance with acute opioid withdrawal management, initiation of Medication Assisted Therapy (e.g., Suboxone [buprenorphine-naloxone]), and linkage to local Recovery Community Organizations for help with outpatient follow up.
